Adding Questions to your Teaching Survey

Instructions for selecting questions and adding custom questions to your end-of-term surveys

  • Standard survey forms are used for each school or department. The current forms have been approved by the school or department and cannot be altered.
  • Faculty may select questions from a question bank and/or add custom questions (our question library has many categories to choose from).
  • Question selections must be saved and submitted, or they will not appear on surveys.

Faculty will receive an email notification when the option to add questions is open. Questions may be added up until the day before surveys begin. Questions can be copied from one class to another within the current term. If you do not wish to add questions – no action is necessary. Forms may be submitted with no changes to avoid receiving reminder notifications.

Instructions for selecting questions and adding custom questions to your midterm surveys

Follow these instructions if you:

  • Chose to write your own midterm survey questions.
  • Want to add questions to the standard set of OMET midterm survey questions.
    • You may choose questions from the question bank or write your own. View our question library for samples and suggestions.
    • Custom questions are either scalable (strongly disagree to strongly agree) or open ended.
    • For multiple instructor classes:
      • The OMET standard set of questions are course specific and results to those questions are released to all faculty.
      • Custom questions added by faculty or where faculty opt out of the OMET standard set and choose to write their own are instructor based and therefore each instructor must add their own unique questions and will be the only one to receive results to those questions. This applies even if the question is worded as course related, i.e. “I can find course materials easily.”
      • If you are teaching more than one class, you may copy questions selected for one course for use in another course.
      • If you do not wish to add questions to the standard OMET question set – no action is necessary. To avoid receiving reminders, simply click the “Submit” button without creating any questions.

IMPORTANT – Make sure to Submit your changes or they will not be added to the survey.

  • Chose to write your own pre-course survey questions.
  • Want to add questions to the standard set of OMET pre-course survey questions.
    • We have samples and suggestions or you can write your own.
    • Custom questions are either scalable (strongly disagree to strongly agree) or open ended.
    • For multiple instructor classes:
      • The OMET standard set of questions are course specific and results to those questions are released to all faculty.
      • Custom questions added by faculty or where faculty opt out of the OMET standard set and choose to write their own are instructor based and therefore each instructor must add their own unique questions and will be the only one to receive results to those questions. This applies even if the question is worded as course related, i.e. “I can find course materials easily.”

Section 1: Overview:

Go to your teaching survey dashboard and open the “Choose the personalized questions” task.

This section provides an overview of the new Question Personalization option page layout. Many of the features are inherent to the software, please go to Section 2 to review available options.

  1. Table of contents – Provides a simplified overview of question sections you can personalize. The page will automatically scroll to the question section you selected.
  2. Filter – Filters the list of questions displayed.
    • View All – All available questions.
    • Selectable – Only questions that can be selected.
    • Customizable – Only personalized questions that can be modified.
    • Included – Only questions that have been chosen to be included in the questionnaire.
    • Excluded – Only questions that have been chosen to be excluded from the questionnaire.
  3. Preview – Allows you to preview a sample of your evaluation from the perspective of your students. Make sure to save your changes before using the preview function.
  4. Save – Saves your changes on the page without submitting.
  5. Submit – Submits your personalized questions to be included in the evaluation.
  6. Copy options – Allows you to copy selections and personalized questions to or from another course.

Section 2 – Adding Selectable Question Bank items

G. Go to the “Select your questions from the question bank” option

  1. i, ii, iii – open the drop-down options to view questions you may add to the survey. There are scaled and open-ended question options.
  2. iv – select include to add the questions to your survey
  3. Make sure to SAVE, PREVIEW, AND SUBMIT your changes OR go to Section 3 for instructions on how to add custom worded questions.

Section 3 – Adding Custom Worded Questions

H. Go to the “Create custom-worded questions” option:

  1. i, ii – open the drop-down options to view questions you may add to the survey. There are scaled and open-ended question options.
  2. Choose Customize (iii) to open the text box where you may type in your question. (Scale options are not customizable.)

  3. Repeat these steps to add open-ended custom questions.
  4. Remember to SAVE, PREVIEW, AND SUBMIT your changes.

Section 4 – Copying questions between your courses

If you have multiple courses, you may be able to copy personalized questions between your courses if allowed by your Blue administrator. This can save you a lot of time and effort in rewriting applicable questions that you have added to previous evaluations.

You will have two copy options:

  • Copy selection from – Allows you to copy questions you have already selected in a different course to this evaluation.
  • Copy selections to – Allows you to copy questions you have selected in this evaluation to one or several of your other courses.

Copy Selection From

If you have already completed a question personalization task for a different course, you can copy the same selections and personalized questions from that course to the one you are currently completing.

  1. Click Copy Options from the Question Personalization menu.
  2. Select Copy Selections From. A window will appear named Copy selections from.
  3. Search for the course name you wish to copy the questions from and select it.

  4. Select to Copy finalize the process.
  5. Review the task with the copied questions before submitting.

Copy selections to

When you have completed a question personalization task, you can copy the selections and personalized questions you made to one or several other courses.

  1. Click Copy Options from the Question Personalization menu.
  2. Select Copy Selections To. A window will appear named Copy selections to.
  3. Search for the course name you wish to copy the questions to and select it.

  4. Choose to only copy the questions, or copy and submit the task immediately.
  5. Select Copy to finalize the process.
    CAUTION: Choosing to copy and submit can save you the step of manually submitting a task for each course one by one. However, this also means you cannot review or modify the questions in the task after it has been submitted.
